What is the Care Connection Team?
The Care Connection Team is a group of health workers who help people outside of the hospital. They work together to help people who are most likely to need to go to the hospital. The team wants to stop these people from having to go to the hospital by giving them the care they need early.
The service aims to:
- Reduce hospital visits
- Reduce follow-up appointments at the hospital
- Free up time for GPs (doctors)
Who is this for and how would I be referred into the service?
The team of health workers in Hillingdon uses special tools and their knowledge of local doctors and hospitals to find patients who need help. They contact these patients directly to arrange visits and provide the necessary care. This way, they make sure everyone gets the right help at the right time.
They focus on:
- Patients over 55 with one or more long-term conditions who might be at a higher risk of going to hospital
- Adult patients under 55 with 3 or more long term conditions who might be at a higher risk of going to hospital
- Patients who have gone to A&E (Accident and Emergency at the hospital) three or more times in three months
Where are the appointments held?
Consultations happen on the telephone and at patients’ homes or care homes. Sometimes the team will visit patients when they are in hospital.
